In the past, the first drugs to treat erectile dysfunction (ED) were sildenafil (Viagra®) and tadalafil (Cialis®). However, more recently, other phosphodiesterase type 5 inhibitors (PDE5i) have become available, including Cialis® and vardenafil (Levitra®). However, there are differences between these drugs in terms of their effect on PDE5 inhibition and in their mode of action, which has made the studies of this drug more complicated. This study investigates how the drugs differ in their effect on the PDE5 activity, a key determinant of blood flow.
In this study, we aimed to compare the effects of sildenafil (Viagra®), tadalafil (Cialis®), and vardenafil (Levitra®) on the activity of PDE5 enzymes in the human corpus cavernosum of patients with normal and benign erectile dysfunction.
This study was conducted as a retrospective analysis using a single-center retrospective study in patients with ED, who were evaluated by a specialist, who was blinded to the study design, for all of the drugs in the study. The study population consisted of patients with ED who underwent a hysterectomy and/or corpus cavernosum resection following the approval of the American Society of Erectile Medicine (ASEM). Patients were assessed for the presence of ED according to the International Index of Erectile Function (IIEF) [1] and their baseline characteristics as shown in.
Sildenafil was used as the reference drug in this study. Patients who did not have an International Index of Erectile Function (IIEF) questionnaire were excluded, and the remaining study group consisted of 441 patients. A total of 522 patients were screened for the presence of ED. Patients were divided into two groups based on the use of sildenafil (Viagra®), tadalafil (Cialis®), and vardenafil (Levitra®) as reference drugs. In addition to the use of sildenafil, we excluded patients who were taking other drugs, such as alpha-blockers, nonsteroidal anti-inflammatory drugs (NSAIDs), antifolate, and vitamin C preparations, for at least 12 hours before the procedure, and those who were prescribed a nitrate medication (e.g., isosorbide dinitrate) for ED treatment, including nitroglycerin. The study protocol was approved by the institutional review board of the University of Basel (protocol number: 2015-05-31-14).
The main objective of this study was to compare the effects of two phosphodiesterase type 5 inhibitors (PDE5i) on the activity of PDE5 enzymes in the human corpus cavernosum of patients with normal and benign erectile dysfunction. The results showed that sildenafil, tadalafil, and vardenafil decreased the activity of PDE5 enzymes in the corpus cavernosum of normal and benign erectile dysfunction patients, and that tadalafil decreased PDE5 enzymes in the corpus cavernosum of men with ED. The mechanism behind the differences in PDE5 activity between the two PDE5 inhibitors may be related to their different pharmacological profiles, which include nitric oxide, cyclic guanosine monophosphate (cGMP), and nitric oxide synthase (NOS).
The study demonstrated that vardenafil, the only PDE5 inhibitor that has been shown to significantly improve penile erection quality in patients with ED, showed significant improvement in erectile function over time (P < 0.001). However, this finding was less significant in the group that received tadalafil or sildenafil. In addition, there was no significant difference in penile hardness, rigidity, or rigidity at baseline (P > 0.05) between the two groups. These results demonstrate that sildenafil, tadalafil, and vardenafil are not the only PDE5 inhibitor with significant improvement in erectile function in men with ED.
